    Hardship/Late Withdrawal Request Form

    To withdraw from a class after the official Last Day to Withdraw due to medical reasons or other hardship.
  • Instructions

    You may withdraw from a class for any reason through the posted Last Day to Withdraw for that class. You can withdraw online in your ctcLink Student Portal, or by emailing registration@lowercolumbia.edu. 

    When is the Last Day to Withdraw?

    • Fall/Winter/Spring term standard classes: End of the 8th week, published on LCC Academic Calendar
    • Summer term and short classes: 80% of the length of the class, calculated and published on the Short Classes Important Dates page

    If your request is submitted before the Last Day to Withdraw, the reason for the withdrawal does not matter and no documentation is required.

    Students who need to withdraw after the Last Day to Withdraw should submit this Hardship Withdrawal Request Form.

    • Medical

      Definition: A significant injury or illness requiring recuperation which interferes with a student’s ability to be successful in school. 
      Documentation Requirements: Written verification provided by a licensed healthcare practitioner stating the school withdrawal is warranted, or other documentation from a healthcare facility. The verification must include a date of injury or duration of illness.

      Note: Detailed medical information (specific diagnoses, treatments, etc.) is not necessary. Acceptable documentation includes a note from a healthcare provider/facility, a screenshot from a healthcare provider app/website showing summary or visit information, or an email from an LCC department or external agency who has verified the medical issue. 

      Hardship

      Definition: A significant and unanticipated personal emergency or circumstance beyond the control of the petitioner.
      Required Documentation: Documentation specifying the date of the personal emergency or circumstance with sufficient details to support the petition.

      Death

      Definition: A death in the student’s immediate family which includes parent(s), sibling(s), spouse/partner, or dependent.
      Required Documentation: Obituary, death notice, or death certificate showing the student’s relationship to the decedent.

    • ⚠️Death: A death in the student’s immediate family including a parent, sibling, spouse/partner, or child or other dependent. 

      ⚠️Required Documentation: Obituary, death notice, or death certificate showing the student’s relationship to the deceased. 

    • ⚠️Military Service: Orders to report for active duty, training, or other mandatory service assignment for state or federal armed services during the quarter.

      ⚠️Required Documentation: Written military orders. 

    • ⚠️College Error: An inaccuracy in enrollment records resulting from human or administrative error which resulted in the student being charged for a class without the normal opportunity to successfully complete and/or or to withdraw prior to the last day to withdraw.

      ⚠️Required Documentation: Written verification from the staff, faculty, or department head where the error occurred. Screenshots of emails are acceptable.
