Instructor Use: Proctor Request Form
Please complete this form so we can add your exam to our scheduling site. All LCC students are required to schedule their exams in advance, so the sooner we receive your request, the sooner students will be able to book their appointments.
Are you requesting an exam with open availability, or a lab reservation (meaning all of your students test at one set time)
*
Open Availability: Your students can test on multiple days, and/or take the exam at various times throughout the day(s).
Lab Reservation: Your students are all testing at one time on a specific date with us. (Note: Please only submit this option if needed, Nursing courses are prioritized)
